1. Use your smartphone to open the App Store or Google Play.
    

2. Search for MyDataHelps (by CareEvolution Inc.) and download it for free to your smartphone.
   
   
   
   
   
   
   
   
   
   
    

3. Once MyDataHelps downloads, open the app and choose “I have a project code." 
    

4. Access the MyDataHelps instructional e-mail sent to you by the study team and enter the alphabetical code listed in the message.
    

5. Once you enter the alphabetical code, simply register yourself for an account and join the PROMPT study.
